Single index page with age verification

Job Description:
**** I have the need for a quick job with super fast turn around. I need this project done within the next 2 hours. ****
I have a PSD file that contains and index page with a single image. This is just a temporary splash screen for an upcoming web site.
We also need a JS Age Verification page. When the user visits the URL, the age verification needs to pop up. The user needs to enter their birth date. The age verification needs to confirm that they are 21 years of age. If they are, they will be allowed to view the splash page.
Must be able to make both index page and splash screen looks like PSD file.
This will be hosted on an IIS server so it would be best if the solution used HTML, CSS and JS only.
