Consultant on Jira and Confluence Add-ons

We have a couple of projects involving Jira and Confluence from Atlassian Software.
For Jira we are looking for a tool/add-on that allows us to connect Forms that customers can fill out, and that will be attached to the Jira-issue. We have looked at some alternatives on the Atlassian Marketplace, but not sure what to chose.
For Confluence we are looking for a tool that allows us to build up external versioned documentation for our software, where we in our software (SaaS) kan link to this documentation. Here we have also looked at some Add-ons, but again not sure what to choose.
We will look for a consultant that have documented experience in the field, and in their response should be able to mention some alternatives for both, as well as possible experice with these alternatives.
We will chose one consultant, and pay by the hour.
