Build Attendance App/Site

I'm looking for an attendance app to be created. You should be able to select a team from a list of teams, and then choose a date to input. All people should be listed for the date, and options of present, absent, late, excused, and a notes section per person. I would prefer these be checkboxes so they are easy to select on mobile as this will be used primarily on mobile. The data should be saved so that reports can be created. I would like to have basic summary reports per team, date, and person. Something similar to this:

I have no preference how this is built, so long as it can be easily accessed and used on mobile devices.
