add pdf functionality and perform one edit to form

Job Description:

Javascript/php form that needs to produce a pdf from the form and send the pdf as an email. The form works and currently sends an HTML formatted email, but we want to produce a pdf. The codeblock for pdf already exists on the internet, so its combining that block into a working solution. We also have a simple edit to the form.
