Build web-based tool that takes csv input, does calculations and outputs a word document

Overview
I want a tool where I can upload a csv of inputs and get word-document as an output.
The inputs will be between 20-50 rows of information. The output will have roughly 200 pieces of information.
The logic on how to populate the word-document based on the provided inputs is long and advanced but instructions will be exhaustively provided. This logic requires a good deal of database lookups and math.
Formatting must be precise and fit a provided word-doc template.
The tool will be web-based with login/pass functionality.
A database will need to be hit to do calculations on the inputs.
I will maintain this data.
The activity in the tool must be logged.
The tool can be very ‘ugly.’
The description is vague for now, as I am looking for estimations on cost. Will provide clearer project description when accepted, but the functional units are all included above.
