Changes needed to Training Manual

We have a company training manual for our sales agents and we need some changes made to it.
I need to insert two blank pages into our training manual and edit one page. On all three pages we will just insert the pdf copies of our insurance application forms which I have already in pdf format but we want to insert it on the manual's pages in order to show our agents what is important on the forms, etc.
We will need about 5 arrows on each of the above 3 pages pointing to important sections with a little bit of text to say what the arrow is showing. Two of these pages will also need new headers.
We then need to make changes to our index page to reflect the above changes and add the new pages to the index.
This should not take longer than one hour and at the most maybe two hours. Please show me a portfolio of your design work done before and use the letters "MUL" in the first line of your bid to show me that you actually read this project description and that you are not one of those annoying auto-bidders.
