Payment Authorization PDF Form for employees to create their own forms

I work at an elementary school. We are currently very short on substitutes so very frequently we will have teacher assistants or other hourly employees sub for classrooms. When they cover for a class and act as a substitute they are paid an extra $25 per hour. I would like to build a form that lets my employees enter in their information for this extra pay and creates a pdf for them at the end of every week that I can then verify and print out once confirmed. Information they need to fill out includes their ID number, their name, their address, the dates they worked and how many hours they worked as a substitute, the hourly pay, am the budget code used to pay them for this extra work, and their home address. Attached is the current document we use in our district.
