Need an online form created where the information is populated to a PDF application.
Currently have 5 different insurance products (more at a later date) where each product we have a different .PDF application in each state. The information on the application is the same, except that it has a state code #. So figure 50 different PDF files per product.
In addition to the data we need to collect on the online form to populat the PDF, we need to be able to customize an instruction page which would appear as the first page of the application. For example we need to enter in the product name, cost, agents name and contact.
Once the PDF is created our agents will save it to their hard drive and email out. No need currently to store the information in our database.
