website lead gen form saved to database

**Overview**:

Our client has multiple small satellite websites currently hosted on one shared server. All sites utilize the exact same lead generation form that, when submitted, simply email the field values (name, email, phone, city) to a designated email address. We need to simply take the same form values and save them in a central database when the lead gen forms are submitted on their websites. Additionally the client needs access to this database and the ability to generate a simple CSV file for a given time frame containing the form data so that they can import it into their system.

**Objective**:

Retain the field values submitted from the lead generation forms from all satellite sites into a central database where the records can be saved/exported as a CSV so our client can import them into their CRM tool.

**What is needed:**

* Database to retain leads from lead gen forms

* Way for the form data to pass into the a central database.

* Way for our client to access this database and generate a CSV file for a given time frame (the records should have a time stamp)

Also, Two things that need to be added to this description is the ability to delete records from the DB and it should include a secure login (username & pass) to access a web interface.
