Fix my form

I have created a form horizontally but it just turns out all weird. I applied some css and can't seem to get it to look nice.
I need someone to fix the form and then make it email "[login to view URL]". The form is for users to be able to invite there friends. The HTML email is already made, all I need is this form to pick up the person's username and fill in the part that says FROM in the email.
