82702 Recommend-this-page script

Need a recommend-this-page type of script with a twist. Features: - script should automatically recognize the URL of the page the form is used on and generate a unique tracking URL which is inserted into the eMail-template. When the recipient clicks at the tracking URL, he will be transered to the correct page. - script should log all data (Names, email-addresses, date, clicks,...) and should show admin who clicked on the tracking URL. - data should be sortable by URL, date, name, clicked or not, email, name and click through-rate - form should feature a "send copy to myself" check-box - script will allow up to 5 recipients - optional "custom comments" field can be used at admin's choice - all emails sent should be fully personalized, including subject line - all emails should show sender's name and email in the From: and ReplyTo: fields - when "send copy to myself" check-box is checked, sender will get the same message as receiver, with additional header/ footer. - script will redirect to custom thank-you and error pages - professional looking admin interface required - code is well documented - once finished, full resell-rights to the script will go to me It's up to you if you'd like to use Perl or PHP.
