Objective To use the free software to design the EPR  EML, XHTML  Any other related software technology  PHP, ASP, Pearl (Script language)  The file will be held on a CD Description This designing of the EPR will be in three stages: -  The designing of the Front End/Back End (will be on the CD)/  The linking program (this is the script language that will link both the front and back end together. -A Diabetic Patient encounter with a GP(Health Practitional)is preferred PROJECT DISCRIPTION It is often the case that some patients attend to more than one hospital and in addition patients have no access to their health records for a variety of reasons ranging from practicability to security. Given that at present there are almost no barriers for accessing one’s financial information over the world wide web (WWW), to hold back health information from their owners would seems at odds with the NHS’s goal of empowering patients and carers in their ongoing health decisions. As yet there is no universal electronic patient record (EPR) system in use across the NHS. Web access is almost universal in hospitals and soon more than 50% of UK house holds will be on line. In addition web based technologies are free of loyalty fees and are well supported. This project aims to develop a web based EPR system. Our EPR system will use XML, HTML and related technologies to construct the file and its style. The file will be held on CD, which the patient can keep. Some identifying information will be held on a server that only appropriate people can access (GPs and Hospital Doctors). A copy of the CD will also be held on the server. This project aims to develop this system for both a chronic and acute disease scenarios. One possible outcome of this project would be to put a viable proposal, which we will be put to the Trust for further development and deployment
