PROGRAMMING FOR STRIPE API'S IN PHP

We need to create a form where the clients will be able to fill in their card details as well as their personal information. Once the information is submitted, the system will automatically process the payment and give the client a confirmation if it goes through, and let them know if it doesn't go through.
Following fields have to be on the form:
CREDIT CARD NUMBER
3 DIGIT SECURITY CODE (4 DIGITS FOR AMERICAN EXPRESS)
EXP DATE
AMOUNT (MAXIMUM $3000)
CLIENT'S ACCOUNT NUMBER
CLIENT'S FULL NAME
CLIENT'S EMAIL ADDRESS
CLIENT'S PHONE NUMBER
CLIENT'S ZIP CODE (ASSOCIATED WITH THE CREDIT CARD)
We also need a page where we can see all this information, and the transaction results(Approved / Not Approved). We also need the approval code once the payment goes through.
Also, there has to be a configuration file where we can enter the following:
