Chat bots integration

We are an IT services provider. We'd like our clients to use a slack command to open a ticket in our Connectwise system. We thought of using Zapier but are open to any method to make this work. we imagine it like this:
1. a slash command like /sendticket
2. which will open a slack form for the end user to complete
3. the form will include their email, name, subject, description of issue, upload picture option
4. they submit the form via slakc
5. OPTION 1: slack integrates directly to connectwise and we get the data in or
5. OPTION 2: an email is sent into our system.
Option 2 is acceptable since option 1 may be hard to manipulate.
