I have 6 Forms I would like to send out links to clients, i need a secure website that we log into select the name of the entitity we are going to act on behalf of, The Name off the recipient, email of the recipient, the form or forms that we want to send a link for to complete with a covering letter that is editable in the DB, The recipient can start completing the questionaire and can save the progress by entering a password and there email address and return to this later by clicking link in a part saved email notinfication and entering there email and password to continue completing, if not completed and submitted in X No. of days a reminder email will be sent with a 1st reminder covering letter editable in the database, If not completed in an additional Y No. of days they will receive a second reminder. Upon full completing the questionaire, answering all questions and if answering yes to the question they would need to be prompted to provide further details, then they are able to submit the questionaire and they would receive a pdf copy in an email with a confirmation letter editable in the database. A copy would be send to the Emails specified in the database as the administrators in the form of a pdf and an excel document.
All payments for the project will be released upon completion, we will not part release, questionaires are attached and an outline of the process
