Here is what I envision...
Doing Laparoscopic surgeries for 25 years I realized that the instruments are a handicap for performing even simple tasks, leave alone complicated ones which are a struggle.
The reason is that they all lack a "wrist" joint.
Drawing a parallel from Da Vinci robot which of course costs a fortune, I am envisioning a mechanical arrangement which will allow a Surgeon to transmit wrist motion of the surgeon to the tip of the instrument in ergonomic and congruent manner and which is powered by hand rather than actuaters.
I can't do it alone and will need help of a subject matter [login to view URL] I will be able to give inputs and provide user trials.
