Job Description:
We are currently running a online personal training website -> [login to view URL]
We want to ad an additional functionality.
We plan to aquire leads by offering a free 30 minute consultation in which the potential client can book a call and we will call them up personally to explain the services we offer and how we plan to help them lose weight and get in shape.
Example of reference website in which we want to have similar to is this -> [login to view URL]
Please refer to the form in the front of the page.
These are the items that I want to be implemented:
1) [login to view URL] -> This is the first item in the form. It contains 4 options:
*) Fatburning
*) Build Muscle
*) Burn Fat And Build Muscle
*) Other goal
and at the botton there is "Next" button. An option must be chosen before this next button is active
2) [login to view URL] -> This is the 2:nd item, and it asks the user to enter gender "Man/Woman".
3) [login to view URL] -> This is the 3:rd item, this asks for the age range of the client. See the image for available age ranges that I want to be included.
4) [login to view URL] -> this item prompts the user to enter in text what their goal is. The minimum text must be 100 characters atleast.
5) [login to view URL] -> This last item is contains a question and it states "Are you ready to invest time and money in your self to achieve your goals? , YES/NO"
Once the form is submitted, there should be a confirmation page in the website stating that the consultation is now setup and that they will now receive a confirmation email with the link to schedule a date for the meeting.
I want you to generate a thank you email once the form is submitted. The email should contain this link ->

This is the link the user will use to setup the meeting.
